Friend's Dad , an artificial limb baker , back then, used 2_3 speeds , 2 chains..
2nd Hub up top of the seat tube had a 2nd hubshell mounted cog driving the wheel IGH
Jack shaft method .. It worked too ..
S-A makes that sort of hub now , we have adult trikes using those.
