In the pre-digital days this would happen or the graphic designer was only looking at things from a graphics perspective, not product correct. In the digital pre-press world that would be a thin excuse. Dyslexia would not be.
Now, maybe the image was shot with a traditional 4x5 and the transparency flopped at the scanner but still a big miff.
