FWIW: I once rebuilt a wheel with the spokes laced radially where it had been laced 3X previously. One flange broke apart explosively while I was tensioning the wheel. (Seriously- a piece of shrapnel hit me in the head.)

Granted, lacing a 36-hole OMAS hub radially was a bad idea, but I think the previous lacing left divots in the flange which contributed to the failure.
