Coaster brakes will wear out over time. The metal pads have grooves that will wear smooth. But that takes years of use without being properly maintained. They are simple to work on and rebuild. I have never seen one break though.
I have never broken one, but two neighboring kids had them fail way back, in both cases the wheel locked up. So much for downhill terminal velocity.
