IBO, I assume you already have orthotics but... My wife has Morton's Neuroma and her podiatrist, who is also a cyclist, made her special orthotics that are thin and light for her cycling and dress shoes that have a slight hump between the 3rd and 4th metatarsal that relieves the pressure on the nerve - they work. The other key metrics for the cycling shoe is the width of the toe box and ventilation to keep the feet cool. It should be wider so that as the foot gets hot and expands, the shoe does not compress the foot thus squeezing the nerve which is what the hump in the orthotic is trying to prevent. He also made me a set of orthotics to fix arthritic big toes.
