Using a 5mm hex I can take the outer part of the hub off and then remove the rubber seal but what then? Does the simply axle slide out so I can remove/replace the bearings?

I have new ones and I took the rear apart (actually only knocked the axle out a little way) by tapping gently on the end of the axle. I didn't see if the bearing would come out but you should be able to pry up the seal on the bearing. As others have said be careful because this is you only seal against dirt and water.
