Nowadays, many or most tubular riders use sealant. You need a tubular tire with a removable valve core, which is most tubulars except the very cheapest ones. There are several brands of sealant, and if you search you'll find threads on them (Road Cycling subforum) and tests (elsewhere on the internet). Sealant will seal smaller punctures and you just keep riding.
