This is a video that I found to be particularly useful if you want to see how it is done. Maybe you might have missed something that this will help you catch. If the shifting is still poor I would also consider replacing your derailleur hanger as that is the only other thing that I can think of that can cause it to shift irregularly despite proper cable adjustment.
