Maine USA MV statutes clarify that cyclists should be in the rightmost lane of left turn only lanes. If there's only one left turn lane, that's where cyclist should be. No mention of queuing up or filtering forward.

For me, depends on situation. If it's flat or a downhill, where I know I can maintain speed of traffic right through the turn, I will line up. If it's an uphill where I will be slower than traffic, or some other safety issue, I will ride up to the front of the line to make the turn, before MV traffic.
