No controls? No separation of duty? That's bad management more than anything. Development should not have access to Production.
We have 3 seperate environments, he just put in the change for all 3 at the same time. There systematic control where this couldn't be done was years ago, but we really haven't had any major issues so our managers have really let the brakes off.
