Headset is 1" Shimano 105 threaded, steel racing frame.
You should start with seeing what the problem is first. Open up the headset and look inside. I suspect that your bearings are caged and you have some denting of the lower cup (you seem to suspect the same thing). If the bearings are caged, change to loose ones. If they are loose, change to caged. That should solve the problem.

You may, however, need a new headset.
