The campy bearings go inside of the bb30 adapter cups I have, similar to what those do. (so I might've been wrong about the first pic you posted, but they look a lot like that)

the way to think about it is whether the adapter goes into the bearings or the bearings go into the adapter.
anything that goes into the bearings rotates (e.g. the crank arm/spindle when you don't use an adapter, or an adapter+spindle+arm pressed into the bearings).
if the adapter is an extension of the frame (bearings get pressed into the adapter), then only spindle+arm rotate.
