Cracks at the top expander splits in cast aluminum stems. These problems become worse when the stems are not inserted deep enough into the steerer:

I've had several of those cheap cast aluminum "death stems" break off while riding. Both times I was lucky and I safely stopped the bikes (once on mine and once on a customers bike I was road testing). Had a steerer break off too on another bike. No fun...
