Bent hanger is almost certainly the cause. A new hanger isn't automatically straight because the hanger mount can get tweaked (happened on my wife's bike). First thing would be to take it and have the shop tweak the RD alignment. You could also try fiddling with the micro-adjuster to see if its off by a very small amount.

Shifters are basically a yes/no thing. Either it clicks to shift or it doesn't. If it clicks the problem is likely cable or derailuer.
