What is your shifter and derailleur?

One thing that I've noticed degrades shifting is flex in the system, in general from a few places:
  • Jockey Wheel Bushing Flex (newer derailleurs have less lateral movement of the upper jockey wheel (guide pulley)
    I've installed Aluminum Chinese bearing type Jockey Wheels on a couple of derailleurs which helps shifting quite a bit.
  • B-Screw Adjustment. The more chain between the derailleur and the sprockets, the more flex.
  • Derailleur Body Loose
  • Weak Return Spring on Derailleur.
