All it takes is dedicated bike lanes. I would think that some three lane roads (two-way with left turn refuge) might be a bit narrow and you don't really need a gizmo to define a separation zone.
That is subjective. As to the skill level of the cyclist. For a cyclist that can only do 6-10mph. The segregated bike lanes are dandy. But a cyclist going faster than that, should be on the road.
