For soreness, if doesn't go away, then yes. For numbness, yes.

If the numbness and soreness is where you sit, then yes - absolutely. (you didn't say).
Saddles have to fit properly so you are sitting on your "sit bones" aka ischial tuberosities. If the saddle is too narrow, then you will be sitting on all the nerves and blood vessels that run through that area. That's not good or healthy long term. There are saddles with cutouts that can also help with this. If you are not sure what to do, the best advice is to go to a competent bike store (LBS) with a respected bike fitter and figure it out. Most bike stores have some sort of a demo program to help you figure it out too.
Saddle adjustment can also figure into this with respect to saddle tilt up and down and to some degree, fore and aft. Again, a bike fitter can fix this for you.
