One distinct drawback to stem shifters is that if you are male, have the shifter in a gear that has the lever facing toward the seat and you hit something solid (like a car) you run the risk of being castrated. We used tell that to customers who looked like they might be up to changing. At my last race, I was staying in a once high class hotel in an old town in New Hampshire. Walked into the lobby with my bike and gear. An older man, clearly a resident, came up to me and told me that had happened to him. Sobering.
