This is all personal preference but for me, on my commuter bike, I have:

A front light that has a lot of functions
A tail light
Each spoke has three sets of reflective tape
I have reflective tape on my panniers
I have side lights made from runner lights for trucks.

On me, I have a helmet light that is powerful
I use reflective vest at dark or when it is raining
