I had one on an Atala Grand Prix that was a devil to remove, but eventually got it out using the shop version of the Sheldon Brown fixed cup tool and a cheater pipe for added leverage on the wrench:
Yes that is the one. The bike shop broke their wrench, with a cheater bar on it, trying to get it off. They said that it looked like some of the tubing work on the inside of the BB looks like it may be causing it. Not very high end work inside that BB. They put it back together before I could look inside myself. I have not worked up the nerve to tackle cotter cranks.
