When I flat it typically requires two cartridges to inflate.
The first cartridge contributes to the atmosphere, the second inflates the tire.
User error.
p.s. in general it's a good idea to hold the cartridge BELOW the level of the valve. Get the cheap ones and practice a couple so you get the technique down.
