I've done well with Shimano SPD shoes. I also have some Specialized Defrosters which seem to be ok, although at least one rubs my ankles a bit.

I haven't had the width of my feet measured in years. I think most manufactures have chosen to make shoes in a single one-size-fits-all width, or at least the stores only stock one of each size.

Perhaps part of the problem is inventory. For a small bike store that sells 10 different types of shoes, each in 10 sizes, plus stocking some extras, that comes up to 100 to 200 pairs of shoes. Add in 3 sizes, and suddenly one's inventory triples.
