I suspect you are missing either the Cone Locknut washer or the serrated lip washer (anti-rotation) from the right side. Look at an exploded view of the hub and you will see the sequence, which should go: Cone Lock Nut washer, Lock nut, Serrated Washer with tab, then the Axle Nut with indicator.
