I got that but was wondering which manufacturer.

An image of tthe pedal and another of the cleat would be a big help in determining what the possible causes might be.

When LOOK first came out with their white clipless pedal I happened to step down into some soft soil. It allowed me to clip in but tthe only way to clip out was to rotate my foot towards the bicycle instead of a way from it.

Perhaps tthe OP has either a damage pedal (spring or latch plate?) or a damaged cleat. In my experience a worn cleat leets you clip in okay but the fit is loose and/or it's easy to accidently unclip.

I also wonder if tthe tension adjustment on the one pedal is set way too high?
