I've never had my tire pull off the rim like that.

When I'm riding along, I'll start to feel a couple of harsh bumps... usually a sign that my air pressure is falling. I stop.. Check, and fix.

I'm not sure of the pressure, but I probably never drop below 5 or 10 PSI. For your tire to fall all the way off the rim, you'd have to be down to zero.
