You've done nothing that hasn't happened a lot of other times to many bikes that are still riding fine. Flatten the washer best possible. Deburr the crank arm fitting best possible. Reassemble with lube on bolt threads and washer surfaces (but not the axle flats). After riding a few rides re check the bolt's torque (about 30 foot pounds). Andy
