Bikes: Many, and the list changes frequently

Typically caused by the pawls slipping and then slamming into the next capture point in the ball ring (high side) or cup (low side). Reasons are as stated by Andrew above. Put new pawls in and adjust, lube, and shift gently and you'll be ok. There is some chance that the ball cup is worn and is not capturing the pawls effectively, but I think that's a long shot.
