Some of the reasons: Human error in operation. Human error in design. Human error in manufacturing. Terrorism (state-sponsored and otherwise). Drug abuse of operator. Sudden onset of "temporary" mental illness of operator. Neglect and obfuscation of records. N.R.C. guidelines that allow daily radioactive release from all nuclear plants. Thermal pollution of water source.
I am not even getting into the transportation and/or storage. Not even addressing these so-called "new type" reactors that you cite. "...new ones that use a different type of fuel in a different method, are "almost benign" sounds like industry hype, and any thinking person would refuse to accept this as any kind of description or definition of either the hardware or the system used to operate and control it.
