The leak is coming from a tiny hole in the tube, but a worn tire can also make tubes more vulnerable to a sharp piece of glass etc. poking through the tire into the tube. Also a worn tire makes it harder to brake effectively and can make you skid out on a sharp turn or in wet weather.

So replace the tire, and either patch or replace the tube.
