The crank bolt is held "captive" by the cap. As you extract the bolt, the head pushes against the cap (which is threaded to the arm) and pushes the arm off the spindle. The cap is removable because there's no other practical way to get captive bolt into the arm initially. It also makes it possible to replace the crank bolt if necessary.
