The fixing bolt inside the crankarm is supposed to be loose when the arm is removed. Its just a loose bolt and it would fall out out if the larger self extract bolt was not in place on the outside of the arm. If there is play between the arm and spindle, when the bolt is tight, then you have an interface problem.
