I'd say the Purists bottles, but the valve is a pull up, but you could always swap lids.
I only use the Camelbak bottles because I prefer colder water and anything not insulated, pretty much boils in the Phoenix heat. I skip the ice though, and just freeze water in the bottle.
That said...the Tacx Shanti bottle fits most of your requirements, but I don't know anyone that uses Tacx bottles except for the pros who are sponsored by them.
