Need extra wide SPD shoes

I have foot problems, and I need an extra wide SPD shoe. I tried on size 49 shoes in a couple of bike shops, and they're too narrow. I would wear a size 50 if it existed, but I don't think it does, and bike shops are not willing to order large sizes just for me to try them on.

Is there an SPD shoe that offers copious extra room? I need to wiggle my feet around. I'm tightly bound around the toes in the shoes I've tried on.

My feet are the length of a size 11 (European 45 or 46), but even the size 49 shoes are not big enough for me.

Suggestions?
