Shimano Tool # TL-ST01
I can't remember the guy's name at Bicycle Research (BicycleTool.com) but he makes tools in his shop the old-school way and has way more than what's listed at his site. I picked up a re-pop version of an ancient Shimano freewheel removal tool from him and it wound up being better than Shimano's original. At the time it wasn't listed at the site, but I called and asked and he knew exactly what I was talking about.

Price sometimes carries a premium, but he's quick to ship and the tool is better than tools of the same price.
