I think the "wasted" part refers to the fact that going to see your PCP is an unnecessary intermediate and bureaucratic step that is common in HMOs. If you know you have a broken ankle, and have the X-rays to prove it, why can't the PCP refer you over the phone to save you the trip?
I agree with the "wasted" assessment.
