okay, it's nothing too fancy, but for $50, it's good enough. i am 23, honkey, don't smoke, no diseases, etc.
it's called "Individual Blue Access (PPO)." it's through anthem. (my doctor is in their network, jackpot).
office visit - $25
emergency room - 20% (that is, they pay 80%)
urgent care - $50
RX formulary - $15generic/$30brand
RX non-formulary - 50%
inpatient - 20%
outpatient - 20%
mental health - $25
now, the max that i ever have to pay on anything is $1500, so if i get totally obliterated and the bill is $50000, i'm only out $1500. for like $75/month, you can get $1000, and i think, for $112, you can get a $500 deductible.
edit: i am pretty happy with it, but haven't had to use it for much other than a one-off prescription and an office visit. it keeps me, my parents, and my girlfriend from worrying too much. thumbs up.
