I have only experienced one right hook and it was during a 40k TT. I had borrowed an aero helmet and my visibility was a little different than I was used to and a car passed me and then suddenly swung into a driveway. I have no idea how I avoided it but I went slightly left and hoped that she would evacuate the space and she did. It was pretty close. I did also witness a right hook in AZ where the cyclist actually hit the right hooking car but he managed to stay upright after rubbing the rear bumper with his front tire.
It is amazing how quickly a person driving can forget the cyclist that they just passed. Everyone is in their own zone and when it all goes wrong, it really sucks for the vulnerable party.
