Hard to say with no actual hands on exam but I suspect a loose headset adjustment. There needs to be enough spacers and stem stack to allow the top cap to fully draw up the steerer and not bottom out before the stem's pinch bolts are tightened. If the top cap contacts the steerer's top end before the bearing preload is achieved a loose and clunking fork/headset bearing results. Andy.
