I'm not hugging the stem. My hands are on the humps, just north of the brakes. I can brake with my rings and pinkies and shift with my thumb. It is the perfect place for the maximum leverage and convenience. The only possible better position would be the same place, but on the inside of the bar, like so:



The looks of my shifter are not of the highest priority for me.
