I'm so sorry for your troubles and your dad's. If he remembers the insurance company or agent who wrote the insurance, you might be able to trace it. Almost all of the long term care policies I know require that you pay premiums until you become eligible for benefits, so if there is a policy, you may be able to find premium payment records.
