Some possibilities:

1) Spindle designed for Nuovo Record "thick" cups installed with Record "thin" cups.

2) Spindle designed for 68mm shell installed in 70mm shell.

3) Spindle designed for 68mm "thick" cups installed with "thin" cups in 70mm shell.

4) Wrong ball size. Most Campagnolo bottom brackets use 1/4" balls, but some have used 3/16" balls. 3/16" balls used in a cup/spindle combination intended for 1/4" balls may be too loose.
