Call Phil Wood. I can't imagine they don't make them. (Phil Wood is not a factory that makes this, this and this. Phil Wood is a machine shop. They make enough of the common items to keep them in stock and supply stores, the making a special order item is completely not a big deal.)

I contacted them with an axle length and offset I wanted and a couple of questions. It turned out what I wanted to do could be done with stock items and they even had one on hand, but it would just have taken a little more time for them to make it. They are a real pleasure to deal with.
