that cable pinch plate is oriented incorrectly... the tab that is touching should not be in that position...

move it so that that tab is in a flat on the bell crank that keeps the clamp plate from SPINNING when you tighten the allen screw.... it is NOT there to "keep the cable from rolling out", but simply to keep the plate from spinning when tightened.

there is a groove machined into the bell crank that holds the cable in place.
