It was probably one of those ordered-online fixies. The fork and stem would be facing rearward out of the box to prevent damage during shipping. But with DIY assembly (why pay a bike store to do it?), they didn't rotate them before installing the handlebar. Then they didn't know the threading on the left and right pedals was different, so they ended up stripping the cranks.
