OTOH, in contrast to Johnny's suggestion, I have little faith that a PT can help me. Virtually everybody I know sings the praises of PTs, but over the years I've seen several and I always have pretty much the same negative experience - they always give me exercises that I can't do and can't see the point of. Time and again I've told them that what I really need is advice about my position on the bike, and they almost always say that they can do that, but only after I submit to their weird regimen of exercises. In fact, the one time that a PT actually allowed me to get on a bike to talk about bike position, she commented that my knees were too close to the top tube. She gave me exercises to do and told me to try to think about modifying my pedal stroke so that my knees were straighter, but never mentioned arches, inserts, orthotics, etc. I know other people find PTs to be of great help, but I have not.
Find a PT with experience in bicycle fitting. Or find a cycling coach with a background in fitting people with physical problems. If there is a bike racing team in your area, they should be able to recommend someone.
