What caused the chain to go into the wheel?

That event probably bent your RD hanger and/or your RD. Cable adjustments are not going to solve that.

The hanger alignment needs to be check with an appropriate gauge/tool. Replaceable hangers are designed to bend (fail) to prevent damage to the frame and the RD. The RD may still get damaged, and require replacement, but the frame is usually protected.
