Sorry, I don't know the model, but it's a Formula hub "High Flange Alloy, 32H, Rubber Contact". I went on the Formula website but I couldn't find it. The axle diameter is exactly 5 7/8 inches long, the cone head( in the order of how it appears on the link you posted) is 15mm top to bottom, and 13mm across.
