I'm not sure what you're getting at. I've fixed thousands of bikes. I'm sure front wheels have fallen off, but I haven't heard of it happening. It's not on the scale of derailleurs shifting into spokes. It's not that front wheels falling off was common, it's that the consequences are more serious than those of a derailleur in a rear wheel.
