Both sound like you are starting with too much slack in the cable.

Shift to the small front ring so that all tension is off the cable and loosen the pinch bolt. Center the barrel adjuster now.

Then pull the end of the cable with a gloved hand and tighten the pinch bolt. There should be little or no slack in the cable at that point. Start from there.
