How to tell if it is time to replace external bottom bracket?

I’m having a lot of recurrent crank clicking with seated or standing climbing. It is worse standing. I thought the answer was hat I did not grease the axle splines but I adressed this already. It kept the clicking away for one rain ride. Middle and granny ring clicking is present intermittently during mild to moderate climbs and non-stop duing heavy climbs. When I remove the crank and check the bearings they are smooth feeling with no apparent play. Each one is very tight in the frame so I did not remove to clean & regrease - yet.
The external bearing cups are English threaded, greased well & installed in an aluminum Flyte frame.

How long do these BB’s last? Anything else I should be checking?? Thanks!
